Watching the video of the concert shows the chords as:

They ease into this from the end of Daughter.
The song is one big crescendo, so get crazy at the end.

The strumming for this is that he strums every eighth note and gives accents
to beats 1, the + of 2, and 4.  If you no clue what I'm talking about, 
listen to the song for the strumming.
